Please bring with you to the hospital:

• Referral to a psychiatric hospital
• ID card
• A valid insurance card (we accept the validity of the ID card 6 months from the last confirmation) in which the employer noted the payment of contributions to the National Health Fund Department
• Payer’s tax identification number
• Another recent document, e.g. a certificate of contributions paid out by the employer
• Booklet “Register of Medical Services”
• Shoes, slippers, pajamas, change of clothes, toiletries and hygiene products
• Confirmation of the transfer of contributions (in the case of self-employed persons and voluntarily insured persons, old age and disability pensioners) or a pension or old-age pension slip
